About this event

The Prevent T2 lifestyle change program is proven program to prevent or delay type 2 disbetes. A CDC-approved curriculm and trained lifestyle coach. A year -long program with weekly meetings for the first 6 months, then once or twice a month for the second 6 months. It's vitrual.

No. It's free

What are the participant eligibility requirements?

All origram's participants must be 18 years of age or older. A blood test result within one year of participant enrollment. AIC of 5.7 to 6.5; Fasting glucose of 100 to 125 mg/dl; Plasma glucose of 140 to 199 mg/dl.

Is the prediabetes risk test accepted?

Yes. a postive screening for prediabetes based on the Presiabetes Risk Test online at: http://www.cdc.gov/prediabetes/takethetest/.

When does the program begin and end?

Start- January 7th, 2024 to January 7th 2025.
